Medical and Dental Benefits

Effective January 1, 2005, the minimum number of hours worked in order to qualify or re-qualify for benefits increased to 75 hours for three consecutive months. Providers enrolled in benefits or on the waitlist as of February 28, 2005 will continue at the 65 hours per month qualification until they are terminated from benefits or the waitlist. The health and dental plans are currently offered to the first 649 eligible providers that meet the minimum work requirement and apply for the benefits. Enrollment is not automatic.

Any provider, who feels they qualify for the benefits and has not received the enrollment packet, should call the Public Authority benefits line at 707-565-5703.

The health plan offers most non-emergency visits and prescriptions for a $16.50 co-pay.  The dental plan offers 100% diagnostic and preventative care through a defined dental provider organization (DPO). Check Delta Dental's website at www.deltadentalca.org for DPO providers. The providers’ share of the health insurance premium cost is only $16.50 per month and dental is $6 per month.  The provider must pay union dues and their share of the insurance premium(s) to have insurance benefits. 

Eligibility Requirement

Enrollment in the health and dental plan requires that you meet and comply with deadlines.

Correctly submitted timesheets are accepted until the deadline of 5:00 p.m. on the 15th of each month for hours worked the month prior (for insurance verification purposes). Timesheets dropped off at Northpoint do not meet deadline eligibility requirements for benefits.

Are You Without Insurance?

Kaiser has a reduced premium program "STEPS" for members previously enrolled in Kaiser within 6 out of the last 12 months. For more information, call Kaiser at (800)464-4000 or visit their website at www.kp.org to see if you qualify. Kaiser also offers individual plans.

If you do not have medical or dental benefits, call the County of Sonoma Economic Assistance at (565-5200) to see if you qualify for Medi-Cal or CMSP (County Medical Services Program).
